Cancel/No Show

We request that you notify us 24 hours in advance of your scheduled appointment time if you need to cancel or reschedule your appointment.  If you forget about your appointment, this will be considered a “No Show”.  We will be happy to reschedule your appointment.  If no shows become an issue, for example, three or more in a row, you may be limited to same day appointments if available.  To cancel/reschedule an appointment, please call our office at 715-425-0115 from 8:00 am to 5:00pm to speak to a scheduler.  If it is after business hours, please leave a message.

co-pay, co-insurance, deductibles

Copays are amounts that your insurance expects us to collect the date of your service.  Copays are due on the date of your service.  Coinsurance and deductibles are usually calculated when your insurance claim is processed.  We will bill you for any remaining assessed amounts.  For surgery patients, if you have a large amount of your deductible outstanding, we will require a down payment.

running late?

Out of respect for other patients, if you are more than 15 minutes late for your appointment, you may be asked to reschedule your visit.  All attempts will be made to fit you in the schedule.


Specialty supplies such as lid wipes or eye masks must be paid for at the time purchased.

cell phones

We know cell phones are a part of daily life, however, please be considerate of other patients and refrain from using the speaker phone function. 

specialty lenses

All payments for a specialty lens, such as a Toric Lens, must be made prior to your surgery.  If this is not completed, your surgery will be rescheduled.
